Telehealth Informed Consent

This document explains telehealth as provided through Ryo and the consent you give when receiving care. Please read it carefully.

What telehealth is

Telehealth uses secure technology to deliver healthcare without an in-person visit. A licensed provider reviews your information and communicates with you electronically.

Independent providers

Care is delivered by licensed providers of the Medical Group, who exercise independent clinical judgment. Ryo Health, LLC is a technology platform and does not direct or override that judgment.

Asynchronous care

Your consultation may be conducted asynchronously (you complete a questionnaire and a provider reviews it without a live video visit) only where permitted by your state. Where your state requires a real-time (synchronous) method of establishing the patient-provider relationship, a live video visit with a licensed provider is scheduled and completed before any prescription is issued. Your provider may also request follow-up information or a video visit at any time if it is clinically needed.

Limitations and risks

• Some conditions require in-person evaluation; your provider may refer you for one.
• Technology may occasionally fail or delay communication.
• A prescription is issued only when clinically appropriate and is never guaranteed. Individual results vary.

Weight-management treatment and medication risks

If you are treated for weight management, you acknowledge that you have been offered the Important Safety Information, including the class risk of thyroid C-cell tumors and the contraindication for a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), risks in pregnancy, and that compounded medications are not FDA-approved.

Your responsibilities

You are responsible for providing truthful, accurate, and complete information. Treatment decisions depend on it.

Emergencies

Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.

Consent and withdrawal

By using Ryo, you acknowledge that you have read and understood this information and consent to receive care via telehealth. You may withdraw consent at any time, which may mean discontinuing treatment.
